This is the PMHNP Fellowship Program - Graduate Application for limited slots still available for Cohort 2. A matched placement is not guaranteed.

Graduate Fellows must have completed a PMHNP program within 18 months of February 28, 2024 to be eligible.

There are 3 sections to complete for this application: (1) Applicant Information, (2) Career Aspirations/Experience, and (3) Required Documents.

In section 3 you will be asked to provide the following required documents: current CV/resume, MA APRN license, MA Controlled Substance License/DEA or invitation to sit for licensure, National NP Certification, NPI Number (if available), official transcript (can be in process), two letters of recommendation, and a brief personal statement describing your professional goals related to seeking a fellowship with a CHC.
